I too am a strength coach and had avascular necrosis of the femoral
head at age 26 (29 now).  My condition however was caused by severe
trauma.  I
was shot by a sniper in Iraq through the left hip. The damage to the
femur healed somewhat over the five and a half months I spent in an
external fixator.  However, the damage to the blood supply was too
severe and the head of the femur began to wear away. I was forced to
have a total hip replacement about a year later.  Today my strength
has returned to a decent level.  I am able to box squat about 400 lbs
(180 kg) to just above parallel although I think my ortho Dr. would
have a heart attack if he knew.  I do not however run very much.  I
generally stick to the stepmill, elliptical, and the bike for cardio
work.

I did not have the options for the treatments you mentioned because
the degradation of the femoral head was too severe.  Not trying to
frighten you just relating a simular story/condition. Good luck and I
hope you are able to return to a level of performance higher than
before your condition.
